What to collect
Submit the following:
- Certificate of Insurance (COI)
- Policy number
- Coverage limits
- Expiration date
Common issues
- Submitting an expired or incomplete COI
- Coverage limits below payer or state minimums
- The name on your insurance certificate must match your legal business name and Tax ID
